Fayette Regional Hospital

Fayette Regional Hospital is a not-for-profit consulting resource from Purdue University focused on improving care, managing margins and facilitating compliance for our healthcare provider and association clients. PHA consults, coaches, and trains healthcare professionals in all capacities in both hospitals/health systems and physician practices. It has worked with more than 90 Indiana hospitals and assisted more than 2,300 Indiana physicians in the implementation of quality improvements initiatives at their practices. Fayette Regional Health is working with InROC on screening patients with Behavioral Health Disorders, Substance Use Disorder including Opioid Use Disorder and providing community and provider education on removing the stigma OUD and Medication Assisted Treatment.

IU Health Bedford 

IU Health Bedford Hospital is a modern, Joint Commission approved rural health care facility which offers up-to-date technology and medical practices. It also employs a physician group practice, IU Health Southern Indiana Physicians, which is a multi-specialty practice providing medical services in internal medicine, family practice, general surgery, orthopedics, obstetrics and gynecology, and pediatrics. Nurse practitioners in adult and pediatric primary care have been added to expand medical care. Reid Health

Reid Health has provided the people of Richmond and its surrounding communities with quality health care for more than 100 years. We are consistently recognized regionally and nationally for the quality of both the care and the experience. Reid Health is working with InROC on screening patients with Behavioral Health Disorders, Substance Use Disorder including Opioid Use Disorder and providing community and provider education on removing the stigma OUD and Medication Assisted Treatment.

Logansport Memorial Hospital

Harrison County Hospital 

Harrison County Hospital is organized and operated to serve the health care needs, and improve the health, of the people of our community. mdlogix Inc

mdlogix will equip providers in the consortium with a state-of-the-art platform initially created for the Veterans Health Administration and developed further through collaboration with mdlogix and SAMHSA.  HealtheMe is a web-based platform that drives standards-based care paths across healthcare institutions and IT boundaries. The platform also offers clinically validated tools for behavioral health screening, patient management, and population health analytics, allowing providers to heal our communities through a patient-centric approach.

IRHA/IRSCN

 The IRHA is accountable to the Office of Rural Health Policy and the Department of Health and Human Services for the performance and financial aspects of the InROC. The IRHA is coordinating the formation of the InROC Care Coordination Task Force (InROCTF) through the direction of the InROC project director. This is a multifaceted approach among all consortium partners and community consortium partners within each of the three identified counties of Lawrence, Cass, and Fayette, as well as patients providers, and social service organizations. 

White Bark Health Academy

The WhiteBark Health Academy (WBHA) supports all educational efforts necessary to educate community members and stakeholders on Opioid-Use Disorder (OUD), treatment options, methods for preparing for treatment, how to refer, and how to best support individuals in recovery through its dedicated learning management system (LMS) platform. The LMS services provided through WBHA will serve as a useful tool to track education efforts and evaluate retention of the information presented.
