Southern Indiana Community Healthcare partnered with Indiana University’s Center for Rural Engagement to serve chronic disease patients who were non-compliant with disease monitoring. IU School of Nursing students engaged patients through home visits. Offering an effective model for chronic disease monitoring, the experience also highlights opportunities for enhanced inter-professional education.

Objectives:

- generate a plan for in-home visits for rural patients with chronic health conditions that harnesses the resources, faculty and students of Indiana University

- identify potential burdens for rural patients with chronic health conditions that might limit compliance with their treatment regimen

- outline the essential professional partners and services needed to address social determinants of health for rural patients with chronic health conditions
