If you email email@example.com please specify county of residence, your availability for a phone appointment (day and time), and your area of interest: insurance enrollment, SNAP, TANF, or other.
After receiving your e-mail or your scheduling request, one of our licensed navigators will contact you. As your Authorized Representative during the application process, we will assist you to ensure that all the required information and documentation is gathered and submitted with your application. Required documentation may include, but is not limited to:
- Photo ID
- Birth Certificate
- Social Security number
- Financial information such as pay check stubs for the past 30-90 days, self-employment records, award benefit statements such as pensions, Social Security Disability benefits, etc.

Our CKC-IN team is ready to assist with the following:
- Education about the health insurance program you and your family may qualify for
- Enrollment application assistance
- Troubleshoot application issues
- Help you understand your benefits and how to use them
- Answer questions about coverage options, especially Hoosier Healthwise, and the Healthy Indiana Plan (HIP)
- Specialized referrals
Connecting Kids to Coverage - Indiana is a program to connect children and families to health care coverage in Indiana. This effort is funded by a grant from the U.S. Centers for Medicare & Medicaid Services to the Indiana Rural Health Association. Over the next three years, 2019-2022, the goal of the CKC-IN program is to increase the number of Indiana children and families enrolled in health care coverage. Our program will focus on enrollment in the Hoosier Healthwise and the Healthy Indiana Plan (HIP).
