Ensure the RHC address is located in Health Professional Shortage Area (HPSA) and is rural:

- RHC Certification Application Packet:
Choose an accreditation agency for initial survey for the CMS certification. Established clinics may wait for the Indiana Department of Health to do periodic surveys, or they may contract with one of the following:
- Quad A (AAAASF)
- The Compliance Team
