Community Paramedicine Programs

The Indiana Rural Health Association’s (IRHA) Community Paramedicine programs provided programmatic assistance to rural communities that utilized community paramedicine (CP) and mobile integrated health (MIH) to address chronic disease management, maternal/infant health, and hospital readmission prevention services. IRHA interviewed other CP-MIH programs across the country and found that CP-MIH is the future of healthcare delivery, especially in rural areas. It is cost-effective and reduces the burden on the 911 system and emergency departments. IRHA has been one of the pioneers of CP-MIH and will continue to be involved as an advocate for rural Indiana as the State of Indiana formalizes CP-MIH.

Indiana Community Paramedicine (InCP) / Project Sprout

IRHA was awarded a Health Issues and Challenges grant through the Indiana Department of Health (IDOH) from July 1, 2022 – June 30, 2024 to expand the community paramedicine program in White County, Indiana. InCP and the Monticello Fire Department offered three different discipline services: chronic disease management, hospital readmission prevention, and maternal and infant health (Project Sprout).

Community Paramedicine: Maternal and Child Health (MACH)

The Community Paramedicine: Maternal and Child Health (MACH) program existed to expand the Project Swaddle™ model in Crawfordsville, Indiana to other areas of the state. IRHA worked with the Crawfordsville Fire Department and UnitedHealthcare to direct the program, while consulting Franciscan Health Crawfordsville for hospital guidance. This program ran from July 1, 2021 - December 31, 2022.
