The Indiana Transfer of Care (InTOC) Network

"Building a smoother, smarter system for rural patients."

When rural hospitals need to transfer a patient to a higher level of care, the process should be clear, coordinated, and timely. Across rural Indiana, hospitals and EMS teams work hard every day to deliver such timely high-quality care. But that process of transporting a patient to a higher level of care can be influenced by factors outside any single organizations control. And delays occur. Community Health Needs Assessments consistently reveal that these transportation and transfer-related delays have become recuring operational challenges.

The InTOC network was created in response.

InTOC's initial phase of development focuses on three Critical Access Hospitals serving distinct rural communities across Indiana. These hospitals were selected for their leadership, their willingness to collaborate, and their shared interest in improving the transfer experience for patients and providers. Current partners in the InTOC Network are Sullivan County Community Hospital, Putnam County Hospital, and Union Hospital Clinton. The network also has participation from leadership within the Indiana Department of Homeland Security (IDHS) and other valuable transportation providers across the state.
