Jennifer Profitt serves as a Program Director with the Indiana Rural Health Association, where she leads the Indiana Transfer of Care network (InTOC), a state initiative that connects rural health providers, community partners, and care systems to improve care transitions and outcomes. She brings a diverse background as a consultant, scientist, and educator, along with extensive experience and strategic program development in rural health collaboration. Jennifer is committed to strengthening access, continuity of care, and measurable outcomes across rural Indiana through practical solutions and strong statewide partnerships
